How the care level is calculated

The German care level (Pflegegrad) follows from a points system. In the assessment, individual points are awarded in six modules – from mobility to organising daily life. The individual points of each module are added up; the sum falls into one of five point ranges (0 = no impairments, 4 = most severe impairments), and each point range carries weighted points (Section 15(2) and (3) SGB XI with Annex 2).

The modules count to different degrees: mobility 10 per cent, self-care 40 per cent, demands of illness or therapy 20 per cent, daily life and social contacts 15 per cent. Modules 2 and 3 – cognitive and communication abilities, and behaviour and mental health issues – count 15 per cent together: only the higher of the two values applies. The weighted points of all modules add up to the total points, 100 at most.

The calculator leads through the criteria of Annex 1 and forms the sum per module from your answers; if you have the assessment report, enter the sums directly. Modules 7 (activities outside the home) and 8 (housekeeping) do not enter the total points under Annex 2. What the calculator does – and what it does not

Guidance based on the statute

  1. 1It does not replace the assessmentThe individual points are determined in the assessment. The care fund commissions the Medical Service or other independent assessors (Section 18(1) SGB XI) and decides on the application. The decision of the care fund is what counts.
  2. 2Special needs constellationAnyone with a specific, exceptionally high need for help can be assigned to care level 5 even if the total points are below 90 (Section 15(4) SGB XI). The requirements are set out in guidelines of the Medical Service at federal level – the calculator does not cover them.
  3. 3ChildrenFor children, the impairments are compared with children whose development is appropriate for their age (Section 15(6) SGB XI); this comparison is part of the assessment. The self-assessment does not cover it. Children up to 18 months have their own thresholds (Section 15(7)) – the calculator applies them when you enter the points from the report.
  4. 4Individual criteriaAnnex 1 lists 64 criteria across the six modules. The self-assessment asks them in the wording of the annex (in our own translation); in module 5 you choose the frequency of the measures as the annex divides it. What “mostly independent” means in the individual case is set out in the assessment guidelines – your view can differ from the assessor’s.

Frequently asked questions about the care level

Points, modules and thresholds

How many points are needed for which care level?

Care level 1 from 12.5 total points, care level 2 from 27, care level 3 from 47.5, care level 4 from 70 and care level 5 from 90 to 100 total points (Section 15(3) SGB XI). Below 12.5 total points there is no care level.

Why do modules 2 and 3 not both count?

The statute gives both modules one joint weighted point: the higher of the two values (Section 15(3) sentence 2 SGB XI). Together they make up 15 per cent of the total points.

Which module counts the most?

Self-care (module 4) with 40 per cent – up to 40 of the 100 total points. It is followed by the demands of illness or therapy (module 5) with 20 per cent.

Where do I get the points of the modules?

Before the assessment, from the self-assessment: each answer carries the individual points of Annex 1 to Section 15 SGB XI. After the assessment, from the report – it is sent with the decision of the care fund unless you object to it being sent (Section 18c(2) SGB XI).

What applies to children up to 18 months?

With the same total points they are placed one care level higher: care level 2 from 12.5 total points, care level 3 from 27, care level 4 from 47.5 and care level 5 from 70 (Section 15(7) SGB XI).

Is the result binding?

No. The calculator converts your answers or points under Section 15 SGB XI. The care fund decides on the care level on the basis of the assessment by the Medical Service or other independent assessors.
